Your First Visit to Revive Chiropractic Centers

Step One:

All new patients are requested to fill out the new patient paperwork as completely as possible. The doctors really take time to look at the answers to your questions as it helps them to understand your condition, and how to best serve your needs.

Step Two:

Please read, review and sign all forms.

Step Three:

A one-on-one consultation and exam will be scheduled with one of our doctors to discuss your health issues and goals, uncover the layers of past damage and help determine what may be causing your health problems.

To determine if you are the right fit for our office, you will be required to go through an examination and evaluation, including digital postural pictures, x-rays, a functional movement screen, palpation of the spine, etc. These tests help to determine the precise cause of your problem.  When it comes to the importance of your entire health, we don’t guess, we test because to see is to know.

Step Four:

If we accept your case, we require all new patients to attend a “Better Results Faster” Workshop with their spouse or significant other. We have found that 75% of the reasons that patients have problems is due to what they are doing outside our office. So, if we don’t educate you and provide clear Expectations of your responsibilities to helping correct your condition, we won’t successfully fix your problem for good. We will be discussing what you need to do to get the best results in the shortest period of time.

Step Five:

At this time, the doctor will advise you if any additional tests such as MRI’s or laboratory tests that are needed. Also, you will be required to schedule another appointment for a “Report of Findings” with the doctor. The doctor will need time to review your case and make sure that we are the right office for you.  In this report of findings, the doctor will be going over:

  1. What’s causing your problem.
  2. How it’s affecting your health right now.
  3. What is the appropriate course of action that needs to be taken to correct your problem.
  4. What are the time and financial commitments that need to be made to resolve your health problem.

For those reasons, we request that your spouse or significant other attend your “Report of Findings”
